Why It Matters

Long-Term Conditions Need Steady, Local Care

Living with a long-term condition means regular checks, medicines that need adjusting, and someone to call when something changes. Chronic disease management is a structured way of doing that with your GP. It is part of our GP services in Gympie, and it is designed for people who would rather have one clear plan than a series of unrelated appointments.

Our GPs manage conditions such as diabetes, heart disease, asthma and COPD. Where it helps, we also arrange a team of allied health professionals, such as an exercise physiologist, dietitian or diabetes educator, and share one plan between everyone involved in your care.

Mary Valley Family Practice is a new general practice at 48 Reef St, Gympie. We see patients from Gympie, Southside, Monkland and the Mary Valley, so you can keep your care close to home.

Why Choose Us

One Plan, One Team, Your Local GP

Your GP Knows Your Plan

The same GP follows your condition over time, so reviews build on what has already been done rather than starting again.

Written and Shared

You receive a copy of your care plan. With your permission, we share it with the allied health professionals and specialists involved in your care.

Close to Home

Gympie and the Mary Valley have fewer local specialists than the coast. We aim to keep reviews, monitoring and follow-up here in town where possible.

Case Examples

What Chronic Care Can Look Like

Three common situations, and how your GP would approach each one.

Example 1

Newly Diagnosed Type 2 Diabetes

The concern: you have just been told your blood sugar is high and you don’t know what comes next.

What happens: your GP explains your results, starts a care plan, sets goals for HbA1c, and arranges foot and eye checks and follow-up blood tests.

Example 2

Asthma That Keeps Flaring Up

The concern: you are using your reliever more often and waking at night.

What happens: your GP checks your inhaler technique, reviews your asthma action plan, and adjusts your preventer if needed.

Example 3

Heart Risk and Blood Pressure

The concern: your blood pressure has been high at recent checks and your family has a history of heart disease.

What happens: your GP checks your overall cardiovascular risk, discusses medicines and lifestyle changes, and books a review to see how the plan is working.

How It Works

Your Chronic Care Plan, Step by Step

  1. Book a Review

Book online or call us. Tell reception you would like to talk about a long-term condition, so the appointment is long enough.

  2. Bring Your Details

Bring your Medicare card, a list of your medicines, and any recent blood test, lung function or blood pressure results you have.

  3. Build Your Plan

Your GP reviews your condition, sets goals with you, and writes your care plan. Where it helps, we also arrange allied health support.

  4. Review Regularly

You leave with a copy of your plan and a date for follow-up. We check your progress over time and update the plan as your needs change.

Common Questions

Chronic Disease Management FAQs

What is chronic disease management?

It is a structured approach to long-term conditions such as diabetes, heart disease, asthma and COPD. Your GP reviews your condition, sets goals with you, and writes a care plan that is updated over time.

What is a GP Management Plan?

A GP Management Plan is a written plan for managing a chronic or complex condition. It sets out your goals, treatments and the support you need, and is reviewed regularly. Medicare rebates apply to eligible plans and reviews.

What is a Team Care Arrangement?

A Team Care Arrangement brings your GP and allied health professionals together to work on one plan for you. Medicare rebates apply to eligible arrangements, and there is a limit on the number of allied health services covered each calendar year.

Do I need a referral?

No. You can book a chronic care review directly with our GPs. If you need a specialist, your GP will refer you.

How often should I have a review?

It depends on your condition and how well it is controlled. Your GP will tell you when to come back, and many people have a review every few months at first.

Can you help with diabetes care in Gympie?

Yes. We manage type 2 diabetes with regular blood tests, medicine reviews, foot and eye check referrals, and a plan for healthy eating and activity. Our diabetes care is part of this chronic disease service.

Do you manage heart disease and blood pressure?

Yes. We check blood pressure, cholesterol and overall cardiovascular risk, and discuss medicines and lifestyle changes. If you have chest pain or severe breathlessness, call 000.

Do you see patients with asthma or COPD?

Yes. We review asthma action plans and inhaler technique, and manage COPD with lung function testing where needed, inhaler review and pulmonary rehabilitation referrals. For children’s asthma, see our children’s health page.

Can an allied health professional be part of my plan?

Yes, where it helps. Your GP can arrange a Team Care Arrangement with allied health professionals. Ask us at your appointment whether this is right for you.

What should I bring to my review?

Bring your Medicare card, a list of your medicines, your asthma or diabetes diary if you keep one, and any recent test results or blood pressure readings.

How much does a chronic care review cost?

The cost depends on the type and length of the appointment. Please see our Fees and Billing page, or call us for details.
